What Is MARPE?

MARPE stands for Miniscrew-Assisted Rapid Palatal Expansion. It is a palatal expander supported by small temporary anchor screws, called miniscrews or TADs, that attach the appliance directly to the bone of the palate rather than relying only on the teeth.

 Because the force is applied to the bone, MARPE can separate and widen the upper jaw more predictably than older tooth-only expanders, even in patients whose growth plates have started to fuse. The result is a wider upper arch, more room for the tongue, and a stronger foundation for the rest of orthodontic treatment.

MARPE for Adults: A Non-Surgical Alternative

The biggest advantage of MARPE is that it brings real jaw expansion to patients who are past their early growth years. In adolescence, the midpalatal suture is still open, so expansion is straightforward. In adults, that suture has often fused, which historically meant surgical expansion (SARPE) was the only path. 

By anchoring to bone, MARPE can open the suture in many adults without surgery. That means a less invasive procedure, a shorter recovery, and a comfortable, in-office experience. Not every adult is a candidate, and a 3D evaluation tells us for sure, but for the right patient, MARPE is a genuine alternative to the operating room.

3. Gradual activation. You make small, controlled turns at home over several weeks. Each turn gently widens the jaw, and you may notice a temporary gap between the front teeth, which is a normal sign the expansion is working.
4. Stabilization. Once we reach the target width, the appliance stays in place for several months while new bone fills in and stabilizes.
5. Next phase. Most patients then move into braces or Invisalign to finish aligning the smile.
